Acknowledgments:  No one would ever be where they are if not for the people who came before them to help show them the way, and I'm no exception to this rule.  There have been far too many people for me to list individually but please let me offer my heartfelt thanks to you all, and list the institutions that facilitated my training and shaping: Patchogue-Medford High School, U.S. Navy, SUNY Stony Brook, SUNY Brooklyn, "Downstate" College of Medicine, Robert Wood Johnson Department of Family Medicine, Middlesex Health System Department of Family Medicine and the Portland Outpatient Center, my wife and the other Angels I've been meeting up with recently.  Thank you all so very much for helping me to become the caring physician I am today.
